What is the ASQ and why is it important?

The Ages & Stages Questionnaires (ASQ) is a developmental and social-emotional screening tool for children between birth and age 6. This tool helps determine your child's developmental progress and identify delays. Early identification is essential for providing the most effective intervention or monitoring services. How do I complete the ASQ for my child?

You have a few options! 

1. Complete the screening online HERE

2. Call a Family Services Specialist and complete the screening over the phone

3. Complete the screening in person at our Family Resource Center

What is the commitment level of signing up?

There is NO commitment level of signing up. When you contact Help Me Grow, you will be connected with a Family Services Specialist who can connect you to local community resources and agencies to assist you with your family's needs. After this initial connection, we will provide a follow-up contact to see if the connection was successful and if all your needs were met. At any point in time, you can let us know if you would like to stop receiving contact from Help Me Grow.
